Issue Description: Users can’t access the Files tab for a recently created Microsoft Teams channel and receive an error.

More info: Affected users receive a “Failed to reach this app” error message.

Final Update: 19/12/2024 12:02:00 PM – Microsoft have confirmed that the aforementioned default SharePoint Online documents library issue, whereby channel folder creation requests failed as the document library URL was incorrect, was responsible for causing impact. They have manually corrected and updated the document library URL information, and received feedback from affected users that impact has been remediated.

Scope of impact: Your organization was impacted by this event, and any user attempting to access the Files tab for a recently created Teams channels was affected.

Root cause: A SharePoint Online documents library issue, whereby channel folder creation requests failed as the document library URL was incorrect, was responsible for causing impact.

Next Steps: Microsoft are reviewing their SharePoint online channel folder creation request procedures to help reduce or avoid similar impact in the future
