Issue Description : Some users may be unable to create, add, access or open forms in Microsoft Forms.

Scope of impact : Impact was specific to a very limited number of users attempting to perform various actions in Microsoft Forms.

Final Update : 01/06/2022 01:45:00 PM – Microsoft identified that a recent service update, implemented to redistribute user traffic and connections, inadvertently caused user form requests to fail. They have reverted the offending change, and confirmed after a period of monitoring that impact is no longer occurring.

Next steps : Microsoft are reviewing their update procedures to better identify similar issues during their development and testing cycles.
