Issue Description: Users may be intermittently unable to use Microsoft Copilot (Microsoft 365) within Microsoft Teams meetings

More Info: Our service health telemetry indicated that this issue impacted approximately forty percent of Copilot requests made from within Microsoft Teams meetings.

Final Update: 10/02/2025 08:57:00 AM – Microsoft have confirmed that their fix to repair the code regression has fully saturated across the affected environment and remediated the issue.

Scope of impact: Any user leveraging Copilot within Microsoft Teams meetings may be intermittently impacted.

Root cause: A latent code regression revealed by a recent standard service update resulted in impact.

Next Steps: Microsoft are reviewing their code update processes so they can better identify similar code regressions during their testing and development cycles and avoid comparable impact in the future.
