Issue Description : Users may be unable to create new forms within the Microsoft Forms service.

Final Update : 27/09/2023 14:39:00 PM – Microsoft validated that redirecting affected user traffic to alternate infrastructure has remediated impact. Furthermore, this has confirmed the root cause to be a section of database infrastructure not performing as expected and granting users the ability to connect to SQL or create new forms.

Scope of impact : Impact was specific to some users, who were served through the affected infrastructure, attempting to create new forms in Microsoft Forms.

Root cause : A section of database infrastructure wasn’t performing as expected and granting users the ability to connect to SQL or create new forms.

Next steps : Microsoft are reviewing data and trends on the affected database infrastructure to understand why it wasn’t performing as expected, in order to prevent similar impact from happening again.
