Issue Description: Users experience failures when importing solutions for Copilot Studio agents containing unstructured knowledge sources

More info: Impact may occur when importing a solution or when provisioning the knowledge sources for the imported agents.

Users will receive the following error message when importing a solution, “Sql error: Statement conflicted with a constraint…”

Impact is limited to unstructured knowledge sources.

Final Update: 13/04/2026 08:52:00 AM– Following a period of monitoring service heath diagnostics, we have confirmed that the script has propagated to such a degree that the issue has been resolved.

Preliminary Root Cause: A deployment introduced a configuration change which impacted how entity metadata is managed for Copilot Studio agents containing unstructured knowledge sources.

Next Steps: Microsoft are reviewing our service update procedures to avoid similar impact and reviewing our service issue mitigation processes to more quickly restore service in the future.
