Issue Description: Admins are unable to add Multifactor Authentication (MFA) sign-in methods to users.
More Info: Authentication related features that may have experienced impact:
– Admins may have been unable to add Multifactor Authentication (MFA) sign-in methods to users
– Users may have experienced errors when attempting to perform self-service password resets
– Users may have experienced issues when viewing or registering authentication methods in MySignIns
Final Update: 13/06/2025 09:20:00 AM – Microsoft have completed the configuration changes, and telemetry shows that the service has recovered.
Scope of impact: Impact was specific to some users who are located or served through the affected infrastructure. Organizations in the Europe, Middle East, and Africa (EMEA) and Asia Pacific (APAC) regions may have experienced the most impact due to the issue occurring during their local business hours
Root cause: Database infrastructure that processes authentication related requests was not performing within expected thresholds.
Next steps: Microsoft are reviewing why the database infrastructure started to experience impact.
