Issue Description: Users’ calls disconnect with an error when they are made using an extension in the Microsoft Teams web client.

More info: Calls made with extensions may be disconnected with an error that states “BackToBackOfferDeclinedByPeer”. This impacts version 25044.2208.3471.2155 of the Microsoft Teams web client.

Final Update: 13/03/2025 08:22:00 AM – Microsoft have completed the reversion of the code change leading to impact, and they’ve monitored their service health telemetry to confirm that impact is remediated.

Scope of impact: This issue may impact any user when using an extension in the Microsoft Teams web client.

Root cause: A recent code change that intended to improve service performance and reliability introduced an impacting code regression that resulted in impact.

Next steps: Microsoft are reviewing their Microsoft Teams service update procedures pertaining to the code base responsible for facilitating calls made using extensions to prevent similar impact in future updates.
